RF Microneedling in Aesthetic Medicine: Evidence and Safety Rules

Radiofrequency microneedling has rapidly become a heavily promoted aesthetic procedure for treating acne scars, enlarged pores, skin texture irregularities, fine lines, and facial contours. The technique combines mechanical micropuncture with thermal radiofrequency energy delivery through fine needles. While clinical evidence supports efficacy for certain indications, a systematic review of 41 studies published in April 2026 revealed substantial variations between devices and protocols, along with inconsistent reporting of technical parameters and insufficient long-term follow-up data. Although generally well-tolerated, the FDA documented serious complications in October 2025, including burns, scarring, nerve injury, subcutaneous fat loss, and tissue deformity. Outcomes critically depend on specific device type, needle configuration, penetration depth, energy levels, and treatment area. Rather than a simple standardized salon treatment, RF microneedling requires skilled specialist administration, as the margin between controlled tissue remodeling and harmful injury remains narrower than marketing materials suggest.
